This form distinguishes our work from those who apply temporary falsies by stating «precise application of using a single natural eyelash per extension». It also covers risks, compliance, allergies, policies, release of images and liability. Customization is included on the consent form to cover all your specific needs. Keep organized with a Client Mapping work sheet so you have all the relevant details recorded for future reference. I create an individual lash style that is designed for each client according to their needs and preferences. This pack will also include an After Care Instruction Sheet that can be texted or emailed to the client. It can also be customized to include contact information.

The form continues with a waiver and release form that will clearly state what the procedure involves so there are no surprises for you or your client. This form allows you to record the patch test, any symptoms reported and consent to continue with procedure if no symptoms are present. *Reaction will occur in 1-3 days.
